Add the chicken breasts to a pot.
Add the other ingredients. Start by adding garlic parmesan sauce, heavy cream, cream cheese, and then the grated parmesan.
After adding all the ingredients, turn on the stove and on low flame cook them for four hours.
Meanwhile, cook the penne pasta in another pot, as per instructed by the package and set it aside.
After the chicken has been cooked, shred it using a fork or your bare hands. Mix the sauce well.
Add the cooked pasta into the slow cooker and mix it with the chicken mixture.
Plate the pasta onto the serving plate.
Garnish it by grating parmesan cheese over it and by adding basil leaves on top.
